Providing Medicines and Medical Supplies: Q2 2015 Report

Project Goal:
Upgrade of the existing clinic and managing of the clinic according to RMF/WCF’s global standards, improving hygiene, function and safety as well as standard of medical operations; restore community faith in clinic’s operations.

Project Objectives:

  • Human capacity building and upgrade of the clinic for better health care delivery
  • Provide regular Medicines and Medical supplies to the clinic
  • Provide support to existing Medical Personnel
  • Investigate solar electrical supply
  • Borehole for drinking water and water for the clinic
  • Review urgent needs to increase the quantity of patients treated and quality of treatment
  • Prepare larger project quotes for capacity improvement (solar power/borehole drilling)

Summary of RMF/WCF-sponsored activities carried out during the reporting period under each project objective:

  • Treatment of patients at the clinic, focusing on Malaria, Maternal Child Healthcare, and prenatal care and observation
  • Provision of Medical Laboratory services
  • Maternal and Child Health trainings and outreach being conducted for new mothers
  • Immunization of Newborns
  • Continued purchase and delivery of medicines and medical supplies
  • Provision of Dental care services for patients in the community

Results and/or accomplishments achieved during this reporting period:

  • Patient numbers keep going up with the regular supply of medicine, supplies and lab reagents resulting currently in an average of more than 902 per week.
  • A total of 10,825 patients were treated at our clinic during the Second Quarter 2015
  • Weekly immunizations are being conducted.
  • Basic Laboratory Reagents/Facilities:  Basic laboratory tests are being conducted in the clinic to facilitate better health care delivery

Notable project challenges and obstacles:

  • Communication challenges continue to exist between the village and the town as well as to communicate internationally with the RMF staff as there is no cell phone reception and internet access in the area where the Gure Clinic is located.
  • Need for internet facilities for Mr. Salau’s and Dr. Adeniran’s offices; they use internet cafes that are far away.
  • Provision of potable water: borehole quotes have thus far proven to be too expensive; provision of potable water remains a challenge.

Plans for next reporting period:

  • Continued operations of the Gure Model Health Care Clinic
  • Continued supply of medicines and medical supplies
  • Continued staff support and training

Summary of RMF/WCF-sponsored medical supply distribution and use:
RMF is providing medicines and medical supplies to the clinic in 3-month cycles, more often when needed. A comprehensive list of medications is updated according to the needs at the clinic.

SUCCESS STORIES

Patient 1
 
A.S. is a 2 year old girl from Imam’s compound; she presented with complaints of fever, body weakness and poor appetite.
 
O/E: Temp 38oC, RR 24/min, Pulse 98/min.
 
Diagnosis: Malaria
 
RX: IM Quinine ¾ ml/3 days, PCM inj 2ml ST, Chloroquine syr 1bd/3days; PCM syr 1tds/3days, BCO syr 1tds/5days. Mother was advised on balance diet for the child.
 
 
Patient 2
 
M.A., a 28 year old man from Kuburufu village presented with complaints of passage of watery stool, vomiting and body weakness.
 
O/E: Dehydrated, Temp 37OC, Pulse: 96/min, BP: 90/50mmHg
 
Diagnosis: Gastroenteritis; Dysentery
 
RX: IV Normal Saline IL stat, IV Flagyl 500mg stat, IV Cipro 200mg stat, then Flagyl (oral) 400mg tds x 5/7, ORS (2 sachets).
 

Patient 3
 
A.M, a 28 year old woman from Yoruduku village, 11km from Gure town. She presented with complaints of vomiting and passage of mucoid stool.
 
O/E: Lethargic, Temp: 36.8OC, PR: 90/min, BP:100/60mmHg
 
Diagnosis: Gastroenteritis, Dysentery
 
RX: IV Flagyl 500mg 8/day x 48hrs, 1.M Plasil 10mg stat, IV Normal Saline alternate with D/S IL Shily, IV Cipro 200mg 12/day x 48hrs.
 
 
Patient 4
 
A.S. is a 32 year old man from Baturayparu compound in Gure town. He presented with severe back pain trauma, a log of tree was said to have fallen on him while cutting down the tree.
 
O/E: Severe painful distress, PR: 90/min, BP: 130/80mmHg
 
IMP: Severe Trauma to the back
 
RX: IM Diclofenac 75mg stat, IM PCM 600mg stat, Tabs Diclofenac 100mg 6.d x 5/7, Neurogesic cream, Tabs Vit C 2tds/5days
 
 
Patient 5
 
A.D. is a 39 year old Fulani woman from Gberu Village, a distance of 22km from Gure. She presented with complaints of chest pain, cough and difficulty in breathing.
 
O/E: Ill looking, RR: 36/min, Pulse: 86/min, BP: 120/80mmHg. Crepitations in the lower lung base.
 
Diagnosis:: Pneumonia
 
RX: IM Gentamicin 160mg/day x 5 days, Tabs Amoxyl 500mg tds x 1/52, Tabs Vit C 2tds x 5/7, Syrup cough expectorant 10ml tds x 5/7
 

Patient 6

A 45 year old man from Yasikira town, 18km from Gure presented with complaints of body pain, dizziness, fever and poor appetite
 
O/E: Acutely ill-looking, Temp: 38.9OC, Pulse: 98/min, BP: 130/80mmHg
 
Diagnosis: Malaria
 
RX: IV Dextrose/Saline, Flagyl, Penicillin, Paracetamol, Artesunate and Vitamin C.

 

Background

Nigeria has the 4th lowest survival rate of children under five out of 191 countries, a child mortality rate of 140 of 1000, and a maternal mortality rate accounting for 10% of the global burden of maternal deaths.

Real Medicine Foundation has partnered with the Kwara State Ministry of Health, The Nigerian Youth Service Corps and Gure Gwassoro Ward Development Committee to support the long abandoned Gure Model Health Center. Situated near the Nigerian/Benin Republic border, the clinic is the only access to healthcare for a population of 154,376 in the Baruteen Local Government area and its surrounding towns. RMF has been funding facility upgrades, providing medicines, medical equipment, and local staff to increase and strengthen its capacity to deliver best practice western medicine and critical maternal child health care services.
